Review Prize Draw Terms and Conditions

These Terms and Conditions (“Terms”) apply to the Review Prize Draw (“Giveaway”) hosted by VisionFirst (“Sponsor”) for former and current patients of VisionFirst (“Practice”).

Eligibility:

  1. To be eligible for entry into the Giveaway, participants must be former or current patients of My Vision First.
  2. Participants must have a valid Google account to leave a Google review.
  3. Participants must be at least 18 years of age or older.

Giveaway Period:

  1. The Giveaway will run from November 1, 2023, at 12:01 AM (Eastern Standard Time) to November 30, 2023, at 11:59 PM (Eastern Standard Time) (“Giveaway Period”).

Entry Process:

  1. To enter the Giveaway, participants must complete the following steps:
  2. a. Leave a Google review for My Vision First during the Giveaway Period.
  3. b. Send a screenshot of their Google review to .
  4. c. For an additional entry into the Giveaway, participants must share their Google review on their social media profiles and tag @drbryceappelbaum.
  5. Participants may receive a maximum of two entries into the Giveaway, one for leaving a Google review and one for sharing the review on social media and tagging @drbryceappelbaum.

Winner Selection:

  1. The winner will be selected at random from all eligible entries.
  2. The winner will be announced and contacted via the email address provided during entry.

Prizes:

  1. The prize for the Giveaway will be determined by the Sponsor and communicated to the winner.
  2. The prize is non-transferable and may not be exchanged for cash or other alternatives.

Privacy:

  1. By participating in this Giveaway, participants consent to the collection, use, and disclosure of their personal information for the purposes of this promotion.

Publicity

  1. By participating in this Giveaway, participants grant the Sponsor the right to use their name and likeness for promotional and marketing purposes.

General Terms:

  1. The Sponsor reserves the right to modify, suspend, or cancel the Giveaway at any time, for any reason, with or without notice.
  2. The Sponsor is not responsible for any technical issues, errors, or problems that may occur during the Giveaway.
  3. The Sponsor reserves the right to disqualify any participant found to be in violation of these Terms.
  4. These Terms are subject to change without notice.
  5. By participating in the Giveaway, participants agree to these Terms and Conditions.
