For years, you may have assumed that seeing clearly meant seeing well. But 20/20 vision is only a small part of a much bigger picture. Just like a computer screen can appear crisp while the system behind it is glitching, the clarity of what you see doesn’t fully tell us how well your eyes, brain, and body are working together. So what’s going on?

  • Eyesight is the sharpness of what you see.
  • Vision is how your brain processes and responds to what your eye takes in.

And the truth is, most standard eye exams only check eyesight and eye health. What I mean by that is most standard eye exams only check for visual acuity and the structural integrity of the eyes, not how the eyes function in daily life. In fact, only 54% of adults receive a comprehensive eye exam annually, meaning many people are only getting basic screenings that miss deeper functional issues (Health Care Alliance for Patient Safety).

What is Functional Vision?

Functional vision is how your visual system performs in the real world, not just how clearly you can see letters on a chart.

It’s about how well your eyes:

  • track moving objects
  • work together as a team
  • adjust to shift in focus
  • process visual input without fatiguing
  • coordinate with your body and brain for balance, attention, and movement

While eyesight happens in the eyes, vision happens in the brain. It’s a complex, multi-layered process that relies on an extensive network of cortical regions working in harmony.

Neuroscience research has shown that the primate visual system involves dozens of distinct areas in the cortex, and these areas occupy a major portion—over half—of the brain’s cortex as a whole (Van Essen, D.C., Anderson, C.H., & Felleman, D.J. , 1992). This highlights that vision isn’t just “seeing,” it’s a full-brain operation.

That means if even one part of this visual network is underperforming, you can experience real-world issues—like fatigue, headaches, or coordination problems—even if your visual acuity tests out as 20/20.

If that system isn’t functioning properly, you can experience real challenges even if you technically “see 20/20”. Unless you’ve had a functional vision assessment, these challenges are likely to go undetected.

Functional Vision vs Traditional Eye Exams

 What Most Eye Exams Miss and Why That Matters

A traditional eye exam is important, but it’s just one piece of the puzzle. When you go in for your standard vision check, here’s what’s usually measured:

  • Eyesight / visual acuity: how clearly you can see letters or objects at a distance
  • Refraction: what lens power you need to correct blurry vision
  • Eye health: checking for signs of disease or injury in the eyes themselves

These are all valuable pieces of data, BUT they don’t tell us how well your visual system is working as a whole. That’s where a functional vision evaluation comes in. It looks at how efficiently your eyes and brain work together to help you move, focus, learn, and feel confident in your environment. It tests:

  • Eye teaming: do both eyes align and work together as a team, or does one lag behind?
  • Tracking (pursuits, fixations, and saccades): can your eye smoothly follow a moving target or shift quickly & accurately between objects?
  • Accommodation: can your eyes change focus quickly between near and far tasks and hold focus at near for an extended period of time?
  • Visual processing: how quickly and accurately does your brain interpret what your eyes see?
  • Depth perception: can you accurately judge how far away objects are when you are stationary as well as with dynamic movement?
  • Spatial awareness: do you understand where your body is in space in relation to objects around you?
  • Visual-motor integration: can your brain, eyes, and body coordinate smoothly for tasks like handwriting, sports, or driving?

Without these insights, it’s easy to walk away from a “normal” eye exam still struggling and not knowing why.

Symptoms of Functional Vision Problems You Shouldn’t Ignore

Even if you’ve passed your standard eye test

You’ve been told your eyes are fine. So why doesn’t it feel that way? Functional vision problems can be subtle, but their impact is anything but. When your eyes and brain aren’t working together efficiently, everyday tasks become unnecessarily hard and it’s easy to blame yourself, your attention span, or even you or your child’s behavior.

Here are some of the most common symptoms of a functional vision issue:

  • Frequent headaches, especially after reading or screen time
  • Trouble concentrating or staying focused on tasks that require visual attention quickly become exhausting
  • Losing your place while reading, skipping words and/or lines or needing to use a finger to track
  • Clumsiness or poor coordination like bumping into things, knocking objects over,  trouble with sports or feeling off-balance
  • Light sensitivity to all the lights from indoor lights to screen glare to even sunlight feeling overwhelming
  • Double vision, which means you are seeing overlapping or blurry images or images that move and do not hold steady
  • Eye strain or tired eye even when you’re wearing your prescription glasses

These symptoms are often brushed off or misattributed to other causes like attention issues, anxiety, laziness, or “just getting older”. However, the real issue could be hidden in how your visual system is functioning.

And because vision is rooted in brain performance, functional vision problems often impact energy, mood, and confidence too.

Who Do Functional Vision Problems Impact?

Functional vision challenges don’t discriminate, they can show up at any point in life. But because symptoms are often subtle (or misunderstood), the people who struggle the most are often the ones who get overlooked the longest.

Children misdiagnosed with ADD/ADHD or learning delays

Because functional vision problems often aren’t picked up in a standard eye exam AND because their symptoms overlap with other challenges that are often mistaken for something else entirely.Here are some of the most common misinterpretations:

“He just doesn’t like to read.”

When reading is physically exhausting because the eyes can’t track properly or maintain focus, kids often avoid it not because they dislike learning, but because it’s simply too hard.

“She’s just clumsy.”

Poor depth perception and eye teaming can cause trouble with balance, hand-eye coordination, and judging distances. What looks like clumsiness is often unstable two-eye coordination and/or a visual processing challenge underneath.

“Maybe it’s anxiety or attention issues.”
